Excellence in Home Health Clinical Practice

Rationale

The purpose of this award is to recognize a physical therapist or physical therapist assistant for outstanding clinical practice in a home health care setting.

Criteria

To be eligible for this award, the nominee must:

  1. Be a current member of the APTA Home Health Academy
  2. Demonstrate excellence in clinical practice through superior knowledge of physical therapist or physical therapist assistant scope of practice
  3. Demonstrate exceptional patient safety improvement, exemplary patient advocacy, and patient outcomes
  4. Demonstrate excellence in clinical knowledge through development and/or execution of a clinical program applicable to home health physical therapy commitment to innovative, evidenced‐based clinical care, and problem‐solving in the home setting that leads to improved patient outcomes.

Nomination Process

Any current member of the American Physical Therapy Association may nominate a physical therapist or physical therapist assistant who meets the award criteria. Nomination packets should include:

  1. One or more letters of nomination addressed to the APTA Home Health Academy describing how and why the nominee meets the criteria.
  2. At least one letter of endorsement (support statement) should be included with the nominating letter and address some, if not all, of the following:
    1. How does this nominee demonstrate excellence in clinical practice?
    2. Give examples of how this nominee demonstrates superior knowledge of physical therapist or physical therapist assistant scope of practice.
    3. List specific examples of how this nominee demonstrates exceptional patient safety improvement, exemplary patient advocacy, and patient outcomes.
    4. What clinical programs has the nominee developed or executed applicable to home health physical therapy?
    5. Anything else you want to add to support why this nominee should be recognized for outstanding clinical practice in a home health care setting?

Selection Process

The Nominating Committee will review nominations and the chair will present the Committee's recommendation to the APTA Home Health Academy Executive Committee for approval.

Presentation

The award will be presented following the Business Meeting during APTA Combined Sections Meeting, usually held in February of each year.

The Award will consist of a $50 gift certificate for the APTA Learning Center.
